Agreement & Waiver

Culinary Kids makes the safety of its children its utmost priority and strives to maintain a safe learning environment. I acknowledge and understand that each child/the child in my care, in all Culinary Kids classes and activities, will be exposed to and shall make use of cooking equipment and tools in a teaching class environment. I understand, agree and acknowledge that Culinary Kids, its licensee/s and all persons associated with Culinary Kids do not accept any responsibility or liability whatsoever for any loss, injury or damage of any kind which my child/the child in my care may incur during and whilst engaged in any Culinary Kids class and activity. It is understood and expressly agreed that I hereby release, hold harmless and indemnify Culinary Kids, its licensee/s and all persons associated with Culinary Kids from any and all liability of any kind for any loss, damage and/or injury, howsoever arising, incurred in connection with my child’s/ the child in my care’s attendance in all Culinary Kids class and activity. It is also understood that I accept all of the risks inherent in the preparation, cooking, and eating of all and any food which has been prepared during all Culinary Kids class and activity.

Food Allergy/Dietary Restriction Waiver

I have informed, disclosed and advised my child’s/ the child in my care’s Culinary Kids licensee/s in writing of ANY food allergies/or dietary restrictions from which my child/the child in my care may suffer which information I hereby confirm as accurate, current and correct.

Media Release

I hereby consent to, authorise and allow any use and reproduction by Culinary Kids of any and all photographs, audio, sound bites taken or visual recordings collected or recorded of my child/ the child in my care during his/her participation in any Culinary Kids class and activity. I understand, acknowledge and agree that Culinary Kids will own the photographs, audio, sound bites taken and visual recordings and the right/s to use or reproduce such photographs, audio, sound bites taken and visual recordings in any media forum. I understand, acknowledge and agree that Culinary Kids shall also have the right and discretion to edit the photographs, audio, sound bites taken and visual recordings or prepare derivative works, for the purposes of promotion, advertising, and public relations. I agree that such use of all and any photographs, audio, sound bites taken and visual recordings as described hereinabove will be free of charge and will not result in any liability for payment to any person, company or organisation.

Agreement

I, the undersigned as the parent/guardian, hereby provide my consent for my child/the child in my care to participate in all Culinary Kids classes and activities and declare that I have carefully read, fully understand and unconditionally accept this Agreement, Waiver and Media Release. It is also understood and agreed that the participation in all Culinary Kids classes and activities by my child/the child in my care is at his/her own risk and that I, as the parent/guardian, will provide 1 (one) months’ written notice to my child’s/the child in my care’s Culinary Kids licensee in the event that I wish to terminate this Agreement. It is confirmed that by submitting my child’s/the child in my care’s and my personal information (“Personal Information”) to Culinary Kids, irrespective as to how such Personal Information is submitted, I, the undersigned, hereby consent to the collection, collation, processing and storing of such Personal Information as well as the use and disclosure of such Personal Information in accordance with the Protection of Personal Information (“POPI”) Act 4 of 2013.
